アムジェン株式会社（本社：東京、代表取締役社長：スティーブ・スギノ）と株式会社キャンサースキャン（本社：東京、代表取締役：福吉 潤、以下「キャンサースキャン」）は、この度、千葉県佐倉市と「骨粗鬆症疾患啓発の推進に係る連携協力（疾患啓発コモンズ）」に関する協定を締結しました。アムジェン株式会社とキャンサースキャンの骨領域における官民連携事業協定は、大阪府大阪市に続いて、二例目となります。
骨粗鬆症に起因する骨折の多くは、骨の強度が低下し、わずかな外力によって骨折が引き起こされる「脆弱性骨折」です。一度、脆弱性骨折を起こした骨粗鬆症の患者さんは、短期間のうちに次の骨折を連鎖的に引き起こす「二次骨折」のリスクが極めて高くなります。すでに生じた骨折自体が次の骨折の危険因子のひとつとされており、最初の骨折から1年以内に次の骨折を起こした人は23％、5年以内では54％に上るというデータ1もあることから、骨粗鬆症の早期治療は二次骨折予防のための重要な根拠となっています。
この度の三者協定は、一度骨折を経験しているにも関わらず骨粗鬆症の治療が開始されていない方々に向けて二次骨折のリスクの啓発や、二次骨折予防のための受診を促し、骨折による寝たきりやそれに伴う介護負担の軽減を目的とするものです。
千葉県佐倉市では、人口に占める65歳以上の割合を示す高齢化率が31.9％2と全国（28.7％）よりも高く、これまでにも骨粗鬆症による骨折リスクを防いで介護予防につなげることと、早期から骨量を維持して骨粗鬆症の予防につなげる目的で、健康教育や骨粗鬆症検診を実施3していました。こうした経緯から、本事業の実施にあたって、佐倉市とキャンサースキャン、アムジェン株式会社の三者は、佐倉市における疾患啓発、行政と医療の連携支援を行い、地域の高齢化に伴う課題のひとつである「骨折」のうち、特に二次骨折予防のための環境づくりの構築を目指します。
脆弱性骨折について
世界的に、50歳以上の女性3人に1人、男性5人に1人が生涯で骨粗鬆症による脆弱性骨折を起こすとされており、高齢化に伴いこの数はさらに増加する見込みです。また、骨粗鬆症による椎体骨折や大腿骨近位部骨折は、日々の活動に著しく影響し患者さんのQOLを低下させ、救急病棟を含む通院を増やし、治療費の増加につながります4。大腿骨近位部骨折患者さんのその後の生存率は、一般人口より低い推移を示します5。こうした状況にもかかわらず、日本の骨粗鬆症患者さんのうち治療を受けている割合は、わずか20％程度（推定）に過ぎません6,7。また、脆弱性骨折は介護や寝たきりの要因でもあります。官公庁統計を基にした試算では、骨折・転倒を要因とする介護サービス負担は2018年には1.9－2.8兆円、その中でも家族による介護負担は0.8－1.7兆円と、最も大きな要素となっていることが報告されています8。
